This document serves as a “pathway” guide, with tools and practical strategies, for identifying root causes of identified or emerging problems. It also can assist with planning effective interventions to reduce social and environmental barriers to learning as well as enhance home and community strengths. The guide uses a public health approach to identify collective solutions to address community and population-based health and mental health challenges.
